JFCS's case management services aim to help clients achieve their goals and make informed decisions about their lives.

Case Management: Overcoming the Red Tape

Our Case Managers are advocates for clients, dedicated to cutting through red tape and ensuring that they can access the resources they need without unnecessary barriers to live a healthy life. Our Case Managers conduct an initial assessment with the client and family to better understand living circumstances, offer compassionate support, and craft a personalized plan that encompasses a wide range of essential services:

  • Access to Public Benefits: Navigating the intricacies of public benefit programs and access to essential medical services to maintain the well-being of the client with the support they are entitled to.
  • Referrals & Assistance: Connecting the client with additional community resources such as transportation, utility programs, medical supplies, budgeting, and further assistance with maintaining everyday life tasks.
  • Advocacy: Expertise and information about understanding systems, including legal, educational, and immigration rights, ensuring clients are capable of making informed decisions that affect their lives.
  • Housing Support: Guidance through the process of applying for affordable housing options and assistance in finding safe and suitable living arrangements.

JFCS is a licensed mental health agency. Most insurances accepted – Medicaid, Medicare and private insurance. For those without insurance coverage, JFCS has a sliding fee scale.
